Shalom Health Services Clinic


Shalom Health Care Center operates a clinic within the IMSA West school building.  Nurse Practitioner, Brandi Scroggins, is the health care provider who serves in the clinic full-time.  Nurse Brandi is able to provide the same range of services to students that they might receive in any other community medical clinic, including dispensing medication.  However, in order for students to be seen in the clinic, parents must complete and sign a Consent to Treat form and, when necessary, a Consent to Administer Medication form. 

 

Without consent, Nurse Brandi cannot assist your student even when the student is sick or injured.  New consents must be completed each school year.  Complete the consent form available in multiple languages on the school website.  School Nurse – Family Resources – Indiana Math and Science Academy (IMSA) West


Medication Policy:

 

If prescription medicine must be taken by a student during the school day, the following procedures must be followed:


  • The medication is to be sent directly to school from the pharmacy or physician’s office or brought to school by the parent/guardian (not by the student).
  • The medication must be accompanied by a Consent to Administer Medication form signed by the parent or guardian.
  • The medication container must be labeled with the student’s name, name of the medication, dosage instructions, and time of dosage.
  • Only the amount needed for the days that the student will be taking the medication and the times taken during the school day should be provided to the school.  For instance, if the student is taking Amoxycillin three times a day for two weeks, the school should be provided only with the 10 tablets to be taken in the middle of the day at school.
  • Any extra medication will not be sent home with the student but will need to be picked up by the parent or guardian.

 

Students are not allowed to carry or self-administer medication, except in the following circumstances:


  • The student has a potentially life-threatening condition necessitating immediate administration.
  • A Medication Self-Administration form is on file at the school, signed by the student’s parent or guardian, physician, and school administrator.
